Confidentiality
All things shared remain confidential. Exceptions to this policy
include indications of abuse or neglect of vulnerable populations,
self harm, or harm to others. If these exceptions occur, the client
will be aware and involved in the report of these issues to
appropriate authorities.
Fees
Fees for service are based on 50 minute individual sessions and
60 minute groups. The cost exists on a sliding fee scale to meet
the unique financial needs of each client. All fees are expected at
time of services rendered. The agreed upon fee will be charged
for cancellations that occur with less than a 24 hour notice. Cash
or checks are the only forms of payment accepted.
Treatment Process
The treatment process is a collaborative partnership between
social worker and client. The social worker will provide an
assessment and the client will identify goals to be addressed.
Following the assessment phase, an individualized treatment plan
will be created by the social worker and client. The plan will include
identifying problems, goals, and objectives.
